Medicare

Medicare will pay for home health services

if you meet the following criteria:

You are homebound (you cannot leave

your home without the assistance of

someone else or without the use of

crutches, a cane, a walker, wheelchair

or special transportation).

Your condition requires one of the

following skilled services:

Physical Therapy

Speech Therapy

Skilled Nursing

Services must be ordered by

your physician and are medically

necessary.

If hospitalized, contact the hospital's

social worker and request Central Home

Health to be your home health

company.

Central Home Health

is licensed by the

State of Louisiana

and is a

Medicare Certified Agency.
